The combination of historical data and operational experience leads to much more technically accurate and defensible analyses. The equipment experts – people who have intimate knowledge of the equipment in its operating environment and who are sensitive to the associated hazards – are in the best position to know what failure modes occur, to summarize what happens when a failure mode occurs, and to identify what the consequences are. Representation from experienced personnel is one of the working group’s most valuable assets because it is comprised of people who are knowledgeable, objective, and who are prepared (in the case of legacy equipment) to challenge current practices. This team of multidisciplinary equipment experts is led by a facilitator – a specialist in the application of Total Life Cycle Management analyses. Together the Team focuses on formulating technically appropriate solutions. By utilizing the power of the team environment, safety and operating context are integrated into the core of the decision making process. A facilitated group approach necessitates the interaction of different disciplines and professional hierarchy. |
